1/29/2024 EnchantaVaughn Approved QMHPA greeted Ms. Cosby upon arrival to the home setting. Upon arrival to the home setting, Ms. Cosby appeared to be in good mood evident by her overall disposition. QMHPA inquired about her day. Ms. Cosby informed QMHPA that she was having a good day and she learning to implement what QMHPA discussed during a previous session about not allowing her family members to get her depressed and feel as though she is doing something wrong by not catering to their needs. QMHPA informed Ms. Cosby that this session was going to focus on coping with feelings of depression. Ms. Cosby did not present any symptoms of S/I or H/I at the time of the session. QMHPA began the session by inquiring about Ms. Cosbyâs current thoughts, feelings and emotions. QMHPA engaged Ms. Cosby in a discussion surrounding her medication management. QMHPA educated Ms. Cosby on coping mechanism that will assist Ms. Cosby with managing her depression. QMHPA educated Ms. Cosby that the purpose of this activity was to assist her with recognizing her triggers and how they are noticeable at times when she becomes excited. QMHPA explained to Ms. Cosby how her excitement interferes with her train of thought. QMHPA discussed with Ms. Cosby that the purpose of this activity was for Ms. Cosby brainstorm a time when completing a task and thinking through her thoughts then follow up with the best decisions. Ms. Cosby appeared to be receptive to QMHPAâs interventions and services that were provided to her during the session. Ms. Cosby informed QMHPA that she had taken her medication for the day. During the skills building activity Ms. Cosby was able to provide examples of what she utilizes as coping skills to assist her when she is upset. Ms. Cosby discussed with QMHPA as she has in a previous session that she doesnât always know how and when to apply those coping skills because she has already been pushed to that point. During the skills building activity, Ms. Cosby was not able to recognize her anger triggers, but mentioned to QMHPA that she is learning. Ms. Cosby shared with QMHPA that she doesnât make the best decisions all the time when it comes to things that she doesnât know how to handle.
